Needs Statement
There is a critical need for healthcare professionals to master key concepts in value-based care to deliver high-quality patient outcomes. Value-based care, emphasizing quality, provider performance and patient experience, is vital for optimal patient population care. The Centers for Medicare & Medicaid Services (CMS) defines value-based care as healthcare programs focusing on quality, provider performance and patient experience, with "value" reflecting individual patient priorities. CMS aims to have 100% of Traditional Medicare and most Medicaid beneficiaries in accountable care relationships by 2030. This Continuing Medical Education (CME) activity addresses this need by teaching learners essential skills for success in value-based care programs. Key areas include Transitional Care Management for seamless inpatient-to-community transitions, Hierarchical Condition Category (HCC) Documentation and Coding to accurately convey patient complexity, Readmission Reduction strategies and optimizing ED Utilization and Patient Access. Learners will also explore effective Care Management through Chronic Care and Advance Care Planning, enhancing Patient Engagement, addressing Social Determinants of Health and mastering Quality Metrics in Value-Based Care. This education empowers healthcare professionals to improve patient care, meet CMS goals and excel in value-based care environments.
