Meeting the Needs of Older Adults in the Community: Accessing Services and Supports

Needs Statement

Clinical care providers need to increase utilization of programming and services from the Harris County Area Agency on Aging (AAA) to address social determinants of health for older adults. Providers may be unaware of resources to assist older adults. This activity will  highlight the needs of older adults and how to use available community-based resources to meet those needs.

Target Audience

Physicians, Nurses, Pharmacists, Psychologists, and Social Workers

Learning Objectives

At the conclusion of the conference, participants should be able to:

  • Determine the specific needs of individuals related to social determinants of health
  • Describe the services provided by Harris County Area Agency on Aging (AAA)
  • Access community resources provided by the AAA to address social determinants of health

Activity Evaluation

Evaluation by questionnaire will address program content, presentation, and possible bias.
