The South East Texas Geriatric Workforce Enhancement Program (SETxGWEP), was funded in 2019 by the United States’ Health Resources Services Administration to prepare the Southeast Texas region with the training and resources to address the specific concerns of aging adults and their caregivers. Needs Statement

Clinical care providers need to increase utilization of programming and services from the Harris County Area Agency on Aging (AAA) to address social determinants of health for older adults. Providers may be unaware of resources to assist older adults. This activity will highlight the needs of older adults and how to use available community-based resources to meet those needs.

Target Audience

Physicians, Nurses, Pharmacists, Psychologists, and Social Workers.

Educational Objectives

At the conclusion of this activity, the participant will be able to: 

  • Determine the specific needs of individuals related to social determinants of health.
  • Describe the services provided by Harris County Area Agency on Aging (AAA).
  • Access community resources provided by the AAA to address social determinants of health.
