Needs Statement:

There is a critical need to enhance patient outcomes through effective home health services. Limited provider understanding of home health eligibility, service types and recertification best practices compromises patient care and efficiency. This educational activity for Physicians and Advanced Practice Professionals addresses these gaps. Participants will learn Centers for Medicare & Medicaid Services Home Health eligibility criteria, differences between private home care and home health care and best practices for recertification, discontinuation and ensuring ongoing medical necessity. Through this education, learners will improve patient care, reduce costs and deliver high-quality services.

Physicians and Advance Practice Providers (APPs), Medical Assistants, Nursing, Office Managers

Educational Objectives:

At the conclusion of the session, the participants should be able to:

  • Define the Centers for Medicare & Medicaid Services (CMS) Home Health eligibility criteria and which services Medicare does not pay for.
  • Describe the differences between private home care and home health care, including what services are provided under each care type.
  • Utilize best practices for home health recertification and discontinuation, ensuring ongoing medical necessity.
