Needs Statement:  

In healthcare, providers face a daily challenge in the balance of acute care and care of chronic conditions. Providers are well aware of how to deliver high quality care to their patient population.  The Center for Medicare and Medicaid Services (CMS) and other payers have a standard set of metrics that they request providers to meet each year in the care of their patients. This CME will cover the metric definitions as well as describe the workflows for capturing the required elements in the EHR.

Learning Objectives

At the conclusion of the session, the participants should be able to:

  • Describe the quality metrics required by the Centers for Medicare & Medicaid Services and their role in patient care.
  • Identify quality metrics used by St. Joseph Medical Group and Baylor St. Luke’s Medical Group.
  • Apply strategies to help patients achieve goals for a subset of key quality measures.
  • Identify documentation requirements in the progress note required to meet quality metrics.
  • Identify diagnosis coding required for entry in the Electronic Health Record to meet quality metrics.
