There continues to be a critical need for healthcare professionals to master key concepts in value-based care to deliver high quality patient outcomes. Value-Based care shifts payment from volume (fee-for-service) to quality and outcomes, requiring new skills in care coordination, prevention, data use, patient engagement, and managing costs, ultimately leading to better health, lower costs, and improved patient/provider satisfaction, but success hinges on understanding these complex, evolving models. Value-Based care shifts payment from volume (fee-for-service) to quality and outcomes, requiring new skills in care coordination, prevention, data use, patient engagement, and managing costs, ultimately leading to better health, lower costs, and improved patient/provider satisfaction, but success hinges on understanding these complex, evolving models. Without training, providers risk financial penalties, inefficient care, and failing to meet patient needs in this new system. The Centers for Medicare & Medicaid Services (CMS) defines value-based care as healthcare programs focusing on quality, provider performance and patient experience, with "value" reflecting individual patient priorities. CMS aims to have 100% of Traditional Medicare and most Medicaid beneficiaries in accountable care relationships by 2030. This Continuing Medical Education (CME) activity addresses this need by teaching learners essential skills for success in value-based care programs. Key areas include Transitional Care Management for seamless inpatient-to-community transitions, Hierarchical Condition Category (HCC) Documentation and Coding to accurately convey patient complexity, Readmission Reduction strategies and optimizing ED Utilization and Patient Access. Learners will also explore effective Care Management through Chronic Care and Advance Care Planning, enhancing Patient Engagement, addressing Social Determinants of Health and mastering Quality Metrics in Value-Based Care. This education empowers healthcare professionals to improve patient care, meet CMS goals and excel in value-based care environments.

At the conclusion of the session, the participants should be able to:
- Describe the key elements required for success in value-based care contract.
- Implement strategies to improve performance in value-based care agreements including transitional care management, HCC documentation and coding, and utilization management.
- Utilize best practices for managing patients post discharge to prevent avoidable readmissions
