NEEDS STATEMENT
Common barriers primary care physicians face when referring patients to specialty services include gaps in understanding criteria for referral, access to specialized resources, and coordination of multidisciplinary care. This activity is designed to bridge the professional practice gaps of primary care physicians and subspecialists by enhancing their knowledge of referral pathways, available resources, and collaborative care strategies. Topics may cover available pathways and resources available at Baylor College of Medicine across the specialty disciplines. It will introduce learners to those in the network so that they become aware of who to refer to and what resources are available.
The program will feature expert-led discussions, case studies, small group and q&a sessions with specialists, focusing on the latest guidelines, referral criteria, and best practices for optimizing patient outcomes. The activity will foster connections between primary care providers and specialty teams, encouraging direct communication and collaboration to enhance continuity of care.
TARGET AUDIENCE
This activity is intended for Primary Care Physicians and Subspecialty Groups.
EDUCATIONAL OBJECTIVES
At the conclusion of this activity, the participant should be able to:
- Identify at least two key challenges in diagnosing and managing patients with complex medical conditions.
- Describe the roles and responsibilities of at least two healthcare specialties involved in the collaborative care of patients with complex conditions.
- Summarize current evidence-based guidelines and best practices for the interprofessional management of complex medical conditions.
- Recognize common barriers to effective interprofessional communication and collaboration in clinical practice.
EDUCATIONAL METHODS
Lecture, Case Study, Q&A, and Small Group Discussions
EVALUATION
Evaluation by questionnaire will address program content, presentation, and possible bias.
