Target Audience
This activity is designed for faculty, fellows, residents, medical students, and other healthcare providers who are involved in internal medicine, pediatrics, or transition medicine.
Needs Statement

Healthcare providers require ongoing education to develop clinical expertise in caring for patients with complex needs that span childhood and adulthood. There is a particular need for training in culturally competent care, understanding the influence of culture and language on health outcomes, and applying evidence-based transition tools. Providers must learn to evaluate healthcare system effectiveness in preparing patients and families for transitions, incorporate patient and family voices, and design durable transition plans that address medical equipment, specialist referrals, and self-management strategies. Educational programs should also foster career development for medicine-pediatrics practitioners and promote advocacy for reducing health disparities.

Educational Methods
Educational methods will include lectures and panel discussion.

Objectives

At the conclusion of the sessions, the participants should be able to:

  • Critically evaluate new subspecialty guidelines in internal medicine and pediatrics and incorporate evidence-based recommendations into clinical practice for patients across the age spectrum.
  • Improve interdisciplinary collaboration and deliver culturally and linguistically appropriate care, especially during transitions for patients with chronic childhood conditions moving to adult medical care.
  • Develop and apply comprehensive transition plans that address patient preparedness, durable medical equipment needs, specialist involvement, and self-management strategies, while incorporating feedback from patients and families.
  • Advocate for policy and institutional changes that reduce health disparities and improve care for underserved populations.

Participants may be asked to complete a session evaluation.
