Target Audience
This activity is designed for Baylor College of Medicine faculty, fellows, residents, medical and health professions students, healthcare team members from clinical affiliates (e.g., social workers, psychologists, public health officials), and other individuals interested in issues related to patient engagement and community-centered approaches to improve health outcomes.
Needs

Physicians, residents, fellows, health professions students, and other healthcare providers across all areas of practice require practical education on strategies to effectively engage patients and communities. There is a universal need to deepen understanding of the broader context in which patients live, including access to care, socioeconomic challenges, and cultural or communication barriers. Providers benefit from tools and training to implement patient-centered care, facilitate shared decision-making, and address nonmedical drivers of health. Additionally, education should address organizational and systems-level factors that influence the delivery of community-centered care. This grand rounds series is designed to enhance providers’ awareness of, and ability to implement, strategies that promote meaningful patient engagement, foster trust, and strengthen partnerships with communities. Through case-based discussions, expert insights, and interdisciplinary dialogue, the series aims to equip learners with practical tools and perspectives to address professional practice gaps arising from insufficient attention to patient and community perspectives in care delivery.


Educational Methods

Educational methods will include lectures, case presentations, literature review, and panel discussion.


Objectives

At the conclusion of the sessions, the participants should be able to:

  • Recognize the importance of patient-centered and community-engaged approaches in shaping health outcomes across the lifespan.
  • Describe the role of nonmedical drivers of health across the lifespan.
  • Explain education, research, clinical care, and community outreach solutions for improving patient engagement and community-centered approaches to improve health outcomes.
  • Identify evidence-based practices for addressing policy, quality improvement, and assessment for addressing patient engagement and community centered approaches to improve health outcomes.

Evaluation

Participants may be asked to complete a session evaluation.
