Needs Statement:  

Annual Wellness Visits (AWVs) provide a key opportunity for providers to identify high-risk patients and develop personalized illness prevention plans, including access to immunizations, medication review and advanced care planning. AWVs are provided to Medicare Beneficiaries at no cost, which removes barriers to prevention, helps Medicare Beneficiaries take charge of their own health, and helps providers better prevent illness, detect problems early when treatment works best, and monitor health conditions.  AWVs exhibit improved outcomes, provide measurable quality improvement, and reduce overall cost of care.   Learn more about requirements for the annual wellness visit and how a team based approach may be utilized to reduce the documentation burden on the Provider.

Learning Objectives

At the conclusion of the session, the participants should be able to:

  • Utilize available tools and resources including EHR templates and AWV nurses to capture all required documentation components.
  • Recognize how an AWV can be used as a means to identify quality improvement opportunities and capture quality metrics such immunizations, depression screening, fall risk, and other screenings.
  • Identify which annual wellness visit components can be completed by clinical staff (MA, RN, LVN, etc) and which components need to be completed by a provider.
