Needs Statement:

This initiative aims to educate learners on crucial value-based care concepts, ensuring their success in patient care. Recognizing the significance of value-based care in our healthcare strategy, the CMS has set a goal for widespread implementation by 2030. The CME's goal is to instruct participants on key success elements, including Transitional Care Management and Hierarchical Condition Category Coding. These skills are pivotal in addressing patient transitions and accurately measuring quality and cost metrics. The CME will also cover vital topics like Readmission Reduction, Why Not Home, ED Utilization, Care Management, Advance Care Planning, Patient Engagement, and Social Determinants of Health, contributing to a comprehensive understanding of value-based care.

Target Audience:

This activity is intended for Physicians and Advanced Practice Professionals.

Educational Objectives:

At the conclusion of the session, the participants should be able to:

  • Analyze strategies for success in managing care for patients post-discharge and how the strategies relate to the Centers for Medicare and Medicaid Services (CMS) criteria for translational care management (TCM) visits. 
  • Outline the underlying rationale and significance for hierarchical condition category (HCC) documentation and coding. 
  • Apply key HCCs that need to be re-captured each year for patients in value based care populations
  • Describe the key elements required for success in value based care contracts
