Target Audience and Needs

Geriatricians, primary care physicians, nurse practitioners, physician assistants, and health professionals in related disciplines need to receive regular updates on innovations in geriatric care independent of end-of-life care.  The series aims to optimize the care of older adults who have multiple chronic conditions by demonstrating how documented patient priorities can align with available therapies, diagnostics, and self-care recommendations.  The series also aims is to expand clinical knowledge, increase competence, inform clinical decision-making, enhance the quality of geriatric care, and improve patient outcomes.


Educational Methods

Educational methods will include case presentations, literature review, and panel discussion.


Objectives

At the conclusion of the sessions, the participants should be able to:

  • Review the steps in the Patient Priorities Care process in relation to patient values (what matters), healthcare preferences, and health-outcome goals.
  • Analyze the three strategies that support the alignment of clinical decision-making with patient priorities.
  • Use a real-life example to describe how Patient Priorities Care is implemented in clinical care settings.
  • Implement decisional strategies to align patient care with documented patient priorities.

Evaluation

Participants may be asked to complete a session evaluation.
