Needs Statement:

There is a critical need to educate healthcare providers on the key components of value based care organizations, as well as the performance elements that are necessary for success. This activity is specifically designed for Physicians and Advanced Practice Professionals to address crucial aspects of knowledge and performance enhancement. Participants will explore key areas such as understanding how physicians and APPs contribute to the success of value based care contracts. These include medical management, documentation of patient complexity and quality. Participations will gain broader understanding of these topics and examine the significance in the partnership between Physicians, Advanced Practice Professionals, and the Texas Health Network team through interactive case studies. Additionally, participants will gain an understanding of the Hierarchical condition category (HCC) coding system as well as how it impacts value based care performance.

Educational Objectives:

At the conclusion of the session, the participants should be able to:

• Analyze the roles and applications of Clinically Integrated Networks (CIN), Accountable Care Organizations (ACO), and Medicare Shared Savings Programs (MSSP) within the Texas Health Network Structure, including their impact on healthcare quality, cost, and coordination.
• Describe the value-based care continuum for managed care contracting, including the progression from fee-for-service to population-based payment models.
• Explain how risk is transferred from payers to healthcare providers in value-based care arrangements.
• Define the role of the partnership between physicians, advanced practice professionals, and the Texas Health Network Team in enhancing patient care outcomes. 
• Utilize risk adjustment models to accurately assess and predict healthcare costs and inform resource allocation and planning.
