Needs Statement:

There is a critical need for care management programs, which serve as foundational strategies for advancing value-based care. These initiatives center on proactively identifying patients at high or escalating risk and facilitating their access to resources aimed at enhancing outcomes. The effectiveness of these programs depends on sustained partnership and collaboration across the entire multidisciplinary team of clinicians and support staff. The performance of network physicians and advanced practice providers (APPs) forms the foundation of all value-based contracts. However, a common challenge arises: providers frequently lack awareness of these care management programs, while care management teams may remain uninformed about provider needs.

Target Audience:

This activity is intended for Physicians and Advanced Practice Professionals.

Learning Objectives:

At the conclusion of the session, the participants should be able to:

  • Define care management within value-based care.
  • Describe the critical role of the CIN Care Management Team in coordinating care, improving patient outcomes, and supporting physicians and advanced practice professionals within a value-based care framework.
  • Identify the specific criteria used for directing patients to the CIN Care Management Team.
  • Define how partnership between physicians, advanced practice professionals, and the Care Management Team can enhance patient care outcomes.
