PPC Care Alignment Training for Primary Care Staff
Overview
NEEDS STATEMENT
The process for translating patient priorities into care remains relatively unexplored outside of goals related to symptomatic relief at end-of-life care. To optimize management of adults with multiple chronic conditions, clinicians must be trained to use patient priorities of care to align available therapies, diagnostics, and self-care recommendations with patient priorities.
TARGET AUDIENCE
Physicians, Nurses, Pharmacists, and Social Workers
EDUCATIONAL OBJECTIVES
At the conclusion of the activity, participants should be able to:
- Explain the rationale for patient-centered decision-making strategies
- Apply patients' health priorities to focus healthcare communication and decision-making
- Identify and respond to common challenges that may arise when applying the PPC approach
EDUCATIONAL METHODS
Lecture, Workshop, Role Play, Question and Answer Session
EVALUATION
Evaluation by questionnaire will address program content, presentation, and possible bias.
