Needs Statement:
Social Determinants of Health (SDOH) are the circumstances in which people are born, live, learn, work, play, worship and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. These factos can result in disparities regarding access to nutritious food, housing conditions, education, and exposure to violence or discrimination - all of which have a negative impact on health that results in higher rates of acute and chronic disease, mental illness and substance use. This activity defines and explores the many dimensions of SDOH and the effects of SDOH on population health, how they inform your patient encounters and medical decision making, and the connection between SDOH and health outcomes. Additionally, this activity will define and describe the use for new billing codes that are available to account for the resources required to address Medicare beneficiaries health related social needs.
Learning Objectives:
At the conclusion of the session, the participants should be able to:
- Define social determinants of health (SDOH).
- Comprehend and apply the three new Medicare billing codes that were released to address health related social needs.
- Recognize the relationship between SDOH and health outcomes.
