Needs Statement:  

In healthcare, providers face a daily challenge in the balance of acute care and care of chronic conditions. Providers are well aware of how to deliver high quality care to their patient population.  The Center for Medicare and Medicaid Services (CMS) and other payers have a standard set of metrics that they request providers to meet each year in the care of their patients. This CME will cover the metric definitions as well as describe the workflows for capturing the required elements in the EHR.

Learning Objectives

At the conclusion of the session, the participants should be able to:

  • Identify the quality metrics that the Center for Medicare and Medicaid services requires to be met in the care of their patients.
  • Identify the quality metrics for SJMG and BSLMG.
  • Identify ways to get our patients to goal in a subset of Key Quality Measures.
  • Identify the documentation requirements in the progress note needed to meet these metrics.
  • Identify diagnosis coding needed to enter in the Electronic Health Record in order to meet these metrics.
