Needs Statement:

There is a critical need to ensure a seamless transition of care from admission to the hospital, as it significantly impacts patient outcomes. This activity is specifically designed for Physicians and Advanced Practice Professionals to address crucial aspects of knowledge and performance enhancement. Participants will explore key areas such as understanding the criteria for identifying patients at high risk for hospital readmission, gaining insights into the current state of hospital readmissions within the Texas CIN compared to the broader Texas Market, and recognizing the importance of reducing the likelihood of patient readmission. Moreover, they will examine the significance of the partnership between Physicians, Advanced Practice Professionals, and the Care Management team in reducing hospital readmissions. Through this educational initiative, participants will acquire practical tools to facilitate a seamless transition from hospital to home and effectively refer patients to the CIN Care Management Team when encountering challenges post-acute care hospitalization. This presentation aims to enhance participants' ability to provide coordinated and effective care, ultimately resulting in improved patient outcomes and reduced hospital readmissions.

Learning Objectives:

At the conclusion of the session, the participants should be able to:

  • Define the criteria the CIN Care Management Team uses to focus on reducing hospital readmissions. 
  • Explain the readmissions current state in the Texas CIN relative to the Texas Market 
  • Define the importance of reducing the chance of a patient being readmitted to the hospital. 
  • Define the importance of the partnership of Physicians and Advanced Practice Professionals with the Care Management Team.
