Needs Statement:
Home health services are a key component of patient management and total cost of care reduction when used appropriately. However, providers often lack a comprehensive understanding of home health eligibility criteria. This activity is designed for Physicians and Advanced Practice Professionals to address crucial aspects of knowledge related to home health certification. Participants will explore key areas such as defining home health eligibility criteria, understanding the difference between home health and private home care, and best practices for home healthcare certification.
Learning Objectives:
At the conclusion of the session, the participants should be able to:
- Define the Centers for Medicare and Medicaid Services (CMS) Home Health eligibility criteria and understand what services Medicare does not pay for.
- Utilize the best practices for home health recertification to evaluate patients for ongoing medical necessity and utilize the strategies for discontinuing home health when it is no longer appropriate.
- Describe the difference between private home care and home health care, including what services are provided under each care type.
