Target Audience
This activity is designed for faculty in the Section of Geriatrics and Palliative Care, including physicians, nurse practitioners, and physician assistants, as well as trainees, including geriatric fellows, internal medicine and family medicine residents, physician assistant students, and interdisciplinary team members in clinics and hospitals.
Needs Statement

Older adults, especially those who are medically complex or experiencing diminished functional capacity, require providers with specialized training to help them and their informal caregivers adapt to these changes in ways that promote quality of life parameters. To achieve this, Age Friendly Health Systems are guided by the 4Ms construct: What Matters, Medications, Mentation, and Mobility. Geriatric medicine faculty need to continuously 1) be engaged in professional development on areas of the geriatrics syndrome; 2) conduct clinical research on problematic areas; 3) teach geriatric medicine fellows, residents, physician assistants, nurses, social workers, pharmacists, and rehabilitation specialists; and 4) provide age-appropriate care to patients and their families. Topics on problematic patient care areas and or new procedures and guidelines will be identified by the Planning Committee, and presenters for grand rounds will be chosen who possess the knowledge, skills, and attitudes to inform our faculty and clinicians on these matters.

Educational methods will include lectures and audience response.


Objectives

At the conclusion of the sessions, the participants should be able to:

  • Discuss core geriatric principles, including the identification and management of geriatric syndromes, safe prescribing practices, and strategies for delivering person-centered, interdisciplinary care to older adults with complex medical and social needs.
  • Apply geriatric best practices by integrating evidence-based approaches to manage multimorbidity, deprescribe high-risk medications, address geriatric syndromes (e.g., falls, delirium, frailty), and coordinate care across disciplines to improve outcomes for older adults.
  • Implement geriatric-informed clinical practices by conducting comprehensive assessments, deprescribing potentially inappropriate medications, initiating goals-of-care conversations, and collaborating effectively with interdisciplinary teams to optimize care plans for older adults.

Evaluation

Participants may be asked to complete a session evaluation.
