Target Audience and Needs

Primary care providers, physician assistants, nurse practitioners, social workers, and research faculty from the Community Health Clinics of Harris Health that are part of the Department of Family and Community Medicine need to be regularly updated about evidence-based practices and advances in the field of primary care, particularly with regards to integrated care, subspecialty referrals, self-assessment, and quality and process improvement.  The series aims to increase knowledge of the learners in these topic areas, and to improve their clinical skills and competencies, which will contribute to enhancing the quality of care and improve patient outcomes.


Educational Methods

Educational methods will include lectures, case presentations, and panel discussion.


Objectives

At the conclusion of the sessions, the participants should be able to:

  • Describe contemporary issues confronting primary care physicians and mid-level providers in primary care.
  • Outline recommended evaluations and management of common problems encountered in patients in the primary care setting.
  • Discuss indications for subspecialty referral or consultation.
  • Recognize and interpret a state of clinical emergency and enlist steps for its stabilization.
  • Assess and manage patients with chronic disease in collaboration with other providers in the medical community using an interprofessional and interdisciplinary approach to improve health outcomes.

Evaluation

Participants may be asked to complete a session evaluation.
